Patients may request their medical records directly through Dr. Anderson's office August 1 -15 for a $20 Fee. Utilize the form and send to the office through the email below.
Once received you will be contacted by the office to arrange payment and pick up of your chart
Beginning September 1, 2025, refer to Desert River Solutions to retrieve medical records.
